Home Care Registered Nurse RN
About The Position
HomeWorks, a division of United Methodist Communities, is hiring a part-time Registered Nurse to provide home care in Western/Northern New Jersey. HomeWorks is the home care agency of United Methodist Communities, a New Jersey nonprofit that has served older adults for more than 100 years. The role focuses on building relationships with clients, coordinating nursing services, and promoting positive health outcomes in the home setting. The schedule is 9:00 a.m. to 2:30 p.m., four days per week, with flexibility as needed. The position pays $44 per hour and includes mileage reimbursement for travel between client visits, paid sick time, a 403(b) retirement plan, and paid training and scholarship opportunities. Applicants must hold an active, unencumbered New Jersey RN license or a multistate compact RN license and have 1–2 years of RN experience in home care, home health, hospice, or community nursing. A BSN is preferred. Applicants must be able to produce a negative drug test and may be subject to a background check.
Requirements

Responsibilities
- Complete in-home nursing assessments to evaluate clients’ health, wellness, safety, and support needs.
- Partner with Home Health Aides by providing guidance and helping ensure quality client support.
- Develop and update individualized care plans as clients’ needs change.
- Conduct follow-up visits and evaluations to monitor progress and provide clinical support.
- Build strong relationships with clients and families and serve as a trusted resource.
- Maintain accurate clinical documentation and communicate important updates and changes in condition to nursing leadership.
