About The Position
The Health Care Companion works with the transportation team to accompany participants who need escorts during transportation or to outside appointments. Duties may include assisting with transfers, paperwork, and waiting with participants at appointments. When not escorting participants, the Companion may assist with duties in the Day Center. PACE Health Care Companions also help organize and implement activity programs in the Day Center.
Requirements
- Regular high school degree or equivalent.
- Experience in healthcare as a caregiver or CNA.
- Experience working within an Interdisciplinary Team Model.
- Experience with Electronic Medical Records (EMR).
Nice To Haves
- Experience working with individuals with mental health needs.
- Experience working with the frail or elderly population.
Responsibilities
- Assist transportation staff with loading and unloading participants from vehicles.
- Accompany enrollees to medical and other appointments as needed.
- Assist participants with transfers, toileting, and other needs during transportation and at appointments.
- Assist in the day rooms, kitchen, and personal care areas when not escorting participants.
- Note changes in participants’ physical or emotional health and communicate this information to the Interdisciplinary Team (IDT).
- Monitor participants’ health status and provide a safe environment.
- For PACE Health Care Companions, assist with organizing and implementing activity programs in the Day Center.
