Terros Health is seeking a full-time Family Nurse Practitioner for our Priest Drive Health Center in Tempe, AZ. Requirements

  • Nurse Practitioner AZ License
  • Master’s degree or Doctorate in nursing (Family Nurse Practitioner)
  • Valid BLS certification
  • Valid DEA license and Board Certification (ANCC)
  • Valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy
  • Valid Arizona Fingerprint Clearance card or apply for one within 7 working days of assuming role
  • Must pass a Drug Screen, TB Test and Background Check
  • Proficient using computer-based systems including electronic health records (NextGen preferred), Microsoft Outlook, and other Microsoft Office products.

  • FNP Experience Preferred
  • NextGen Experience is a PLUS
  • Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL language skills (eligible for additional language differential pay)

Responsibilities

  • Conducts physicals/check-ups, medical evaluations, diagnostics, and treats illnesses.
  • Orders lab tests, provides general and preventative care, and prescribes/monitors medications.
  • Coordinates with other providers and makes specialty care referrals.
  • Educates patients about preventative care and establishes therapeutic relationships for treatment goals within an integrated setting.
  • Performs duties independently or as part of a treatment team.
  • Provides leadership and cohesion within the clinic environment, potentially supervising other medical staff and students.
  • Participates in program evaluation procedures and professional record keeping.
  • Makes appropriate referrals for specialty and behavioral health care.
  • Provides direct patient services for at least 85% of the work time.
  • Employs therapeutic interventions appropriate for identified treatment issues.
  • Seeks appropriate clinical supervision/support when necessary.
  • Establishes appropriate patient files conforming to agency guidelines, best professional practice, and State licensing regulations.
