About The Position

One Medical is hiring a Family Nurse Practitioner or Physician Assistant for a hybrid, full-time role based in its West Portal office. The position provides primary care through in-office, testing-center, and occasional telehealth visits. The typical 40-hour weekly schedule includes 24 hours of in-office patient care, 8 hours of remote virtual patient care, and 8 hours for a remote non-patient-facing day. A sign-on bonus and relocation assistance may be available. The clinician manages a patient panel with a broad range of needs, including acute, chronic, and well visits, and participates in ongoing clinical learning and collaboration with in-office and virtual care teams. Providers use their clinical training and may perform in-office procedures. Requirements

Complete an accredited Family Nurse Practitioner or Physician Assistant program and hold national certification.
Within the past five years, have at least two years of experience as an Advanced Practitioner in an outpatient primary care setting seeing patients of all ages (0+); alternatively, have at least one year in that setting plus either a one-year primary care fellowship or at least one year in an urgent care setting seeing patients of all ages (0+).
Hold a California state license by the One Medical start date.

Responsibilities

Manage a patient panel with a broad array of patient needs, conducting acute, chronic, and well visits.
Treat patients in the office or testing centers and conduct occasional telehealth visits.
Participate in continuous learning through weekly Clinical Rounds and other modalities.
Collaborate with in-office teammates through daily huddles and with virtual clinical teams.
Use specific clinical training and take opportunities to perform in-office procedures.
Obtain additional state licensure and credentialing for One Medical virtual primary care in additional states, as needed.
