Family Nurse Practitioner / Adult-Gerontology Nurse Practitioner
About The Position
This is an opportunity for an FNP or AGNP who wants meaningful patient relationships, clinical autonomy, and the ability to take ownership of their practice. You’ll provide primary care to older adults living in memory care, assisted living, and independent living communities throughout Bullhead City. Your day will center on managing the full spectrum of geriatric primary care—from chronic disease and preventive care to acute concerns, medication management, and follow-up visits. Many patients are medically complex, requiring strong clinical judgment, patience, and the ability to balance multiple conditions while keeping quality of life and independence at the center of the care plan. You’ll work with a high level of independence while maintaining access to physician support when needed. You’ll also collaborate closely with facility staff, nurses, therapists, specialists, patients, and families to coordinate care and address changes in health before they become larger problems. For the right provider, this role offers the independence and ownership of building a patient panel and practice style without the administrative burden of owning a private practice.
Requirements
- Licensed Nurse Practitioner with FNP or AGNP preparation and eligibility to practice in Arizona
- DEA registration or ability to obtain
- Strong clinical decision-making and critical-thinking skills
- Excellent interpersonal and communication skills
- Ability to practice independently and collaboratively
- Reliable transportation for local travel between facilities
Nice To Haves
- Two or more years of experience caring for geriatric patients
- Experience in assisted living, memory care, long-term care, internal medicine, family practice, hospice, or palliative care
- Experience managing medically complex older adults
- Familiarity with medication management and care coordination in senior living environments

Responsibilities
- Provide comprehensive primary care to residents of memory care, assisted living, and independent living communities
- Evaluate and manage chronic diseases, acute medical concerns, and medically complex geriatric conditions
- Conduct preventive care, routine assessments, and follow-up visits
- Develop individualized treatment plans focused on health, function, independence, and quality of life
- Manage medications and identify opportunities to reduce unnecessary polypharmacy when clinically appropriate
- Monitor changes in patient condition and adjust treatment plans accordingly
- Collaborate with physicians, nurses, therapists, specialists, facility staff, patients, and families
- Coordinate patient care across facilities, specialists, hospitals, and other healthcare settings
- Educate patients and families regarding diagnoses, medications, treatment plans, and goals of care
- Maintain accurate, complete, and timely documentation in the electronic medical record
- Exercise independent clinical judgment while seeking physician collaboration when appropriate
