About The Position
Community Options, Inc. is seeking a Family Model Provider in the Nashville, Tennessee area. The provider opens their home to an individual with developmental and/or physical disabilities, offering a safe and comfortable home and supporting the individual’s basic physical and emotional needs so they can live a high-quality life. The provider must establish a comfortable, supportive, and genuine relationship with the individual. This independently contracted position provides a 24/7 family-like setting. Providers are reimbursed at a per diem rate and receive monthly room and board from the individual they support; this income is tax exempt.
Requirements
- High school diploma or GED.
- Valid driver’s license.
- Previous experience working as a Direct Support Professional is required.
- Must successfully complete trainings through Community Options.
- Must be open to supervisory visits in your home.
