About The Position

The Full Service Partnership - High Fidelity Wraparound (FSP-HFW) Facilitator coordinates individualized, family-driven, youth-guided, culturally responsive, and strengths-based services for children and youth with complex behavioral health needs and their families or caregivers. The Facilitator works with the child or youth and family or caregiver to identify Child and Family Team (CFT) members and leads the team in developing, implementing, monitoring, and updating the Plan of Care.
