The Encounter Specialist will participate in daily Care Team Huddles to ensure collaborative care for each patient. This role involves reviewing patient medical records for upcoming visits, abstracting pertinent data, and making notes for issues to be addressed during the visit. The specialist will greet and escort patients, obtain vital signs, assess medication regimens for discrepancies, reconcile allergies, and engage patients in conversations about their health. They will prepare charts with necessary data for the provider, enter findings and provider notes into the electronic medical record, and document orders and the plan of care. Additionally, the Encounter Specialist will provide patient instructions and educational materials, inform patients about their next visit, and direct them to check out for scheduling. This role also includes documenting all activities, interventions, and patient responses in the EMR. Other responsibilities may include checking schedules, organizing patient flow, assisting patients with mobility and personal care, collecting specimens, preparing patients for exams, and answering calls. Clerical duties such as sending/receiving medical records, facilitating referrals, completing forms, scheduling appointments, and insurance verification/pre-certification may also be assigned. The specialist is expected to enhance professional growth through in-service meetings and educational programs, adhere to controlled substance regulations, practice universal precautions, and complete training related to HCC coding.
