The Encounter Specialist will participate in daily Care Team Huddles to ensure collaborative care for each patient. This role involves reviewing patient medical records for upcoming visits, abstracting pertinent data, and making notes for discussion during the clinic visit. The specialist will greet and escort patients, obtain vital signs, assess medication regimens, reconcile allergies, and engage patients in conversations about their health. They will prepare charts, enter provider findings, orders, and plans of care into the electronic medical record, and provide patient instructions and educational materials. The role also includes documenting all activities, fulfilling clerical responsibilities such as managing records and scheduling, and enhancing professional growth through education and committee participation. Adherence to controlled substance regulations and universal precautions is required. Training in hierarchical condition category (HCC) coding and competency in transcribing patient visits are also key aspects of this position.
