About The Position
A facility near Yuma, AZ is seeking a Director of Case Management to join their team! This is a full-time, permanent opportunity with a schedule of days, Monday through Friday. The role oversees case management operations, including care coordination, discharge planning, and utilization review. The Director will lead and develop case management staff to support quality outcomes, regulatory compliance, and patient satisfaction. This position also involves collaborating with physicians, nursing leadership, and interdisciplinary teams to optimize patient throughput and resource utilization.
Requirements
- LCSW or RN license
- 3 or more years of hospital based case management experience
- Bachelor's Degree or higher required
Nice To Haves
- Have or be willing to obtain CCM or ACM
