About The Position

A medical facility located near Louisville, KY is seeking a full-time Director of Case Management to join its team. This is a full-time permanent position where the Director will manage the daily operations and HR functions of the Case Management department. The role involves directing interdisciplinary care, discharge planning, and patient outcomes while championing patient advocacy. Additionally, the Director will guide and train staff, build relationships with healthcare and insurance networks, and foster a positive, celebratory work culture.

Requirements

• Current CCM or ACM certification or eligibility to obtain within first year of employment
• Must possess a bachelor's degree
• Three years of hospital-based Case Management experience
• Utilization Review experience
• Discharge Planning experience

• Graduate degree is preferred
