About The Position

Hope Haven is an accredited resource for disability services and advocacy, providing programs and support for children, adults, and families across physical, mental, intellectual, residential, vocational, and spiritual needs. Direct Support Professionals support clients according to their individual goals and help ensure their physical, mental, emotional, and social needs are met. The role serves individuals with intellectual disabilities or chronic mental health conditions.

Requirements

• Good written and verbal communication skills.
• Basic computer skills.
• Ability to exercise sound judgment in making decisions.
• Ability to complete and pass the required medication administration course.
• Valid Class C driver’s license.
• Clearance from all applicable background checks required by service standards.
• Reliable transportation.
• Proof of state minimum liability insurance coverage.

Nice To Haves

• High school graduate or GED/HSED (High School Equivalency Diploma) is preferred.

Responsibilities

• Support individuals’ participation in community life and maximize opportunities to develop relationships with community members.
• Provide support and training identified in each individual’s service plan, and help meet physical and mental needs, including hygiene, bathing or showering, toileting, escort, transition time, and mealtime assistance.
• Teach self-control and self-management skills in accordance with departmental training.
• Complete client logs, charts, and other required documentation according to the service plan and regulations, at the end of the shift or within 24 hours with supervisor approval.
• Support individuals in developing and carrying out daily activity schedules tailored to their needs.
• Help individuals arrange necessary appointments and transportation, and ensure appropriate documentation is completed.
• Promote personal safety and safety measures among team members and clients, and follow all safety procedures.
