About The Position

Learning Services, a Collage Rehabilitation Partner, is seeking a compassionate, dedicated, and proactive Direct Support Professional to support adults with brain injuries in a home-like neurorehabilitation environment in Raleigh, North Carolina. The role involves implementing rehabilitation strategies and providing guidance, support, and encouragement to help clients work toward personal independence. DSPs assist with daily living activities and support vocational, recreational, and community pursuits so clients can build confidence and achieve personalized goals.

Requirements

High school diploma or equivalent.
Valid driver’s license and a record of responsible driving behavior.
Ability to travel and/or operate a vehicle for up to 20% of working time.
Ability to lift up to 50 pounds.

Responsibilities

Assist, guide, and encourage clients with activities of daily living, community living skills, and personalized rehabilitation goals.
Assist with medication administration as assigned, monitor nutrition plans, and prepare well-balanced meals.
Support clients through challenging situations and implement behavior management plans while maintaining professional boundaries and mutual trust.
Observe and report changes in clients’ behavioral, emotional, cognitive, or physical conditions to the treatment team.
Safely transport and accompany clients to medical appointments, group outings, errands, and recreational activities.
Complete required shift documentation, including daily data sheets, goal sheets, vital and seizure flow sheets, supervision checklists, and incident reports, in compliance with CARF, state, and federal standards.
Maintain a clean, organized, safe, and welcoming living environment in alignment with licensing requirements.
Participate in shift hand-offs, review operational communications, and collaborate with family members, care teams, and leadership.
