About The Position

Under the supervision of the Day Habilitation Coordinator, the Direct Support Professional – Day Habilitation ensures quality services for consumers and participates as part of an Interdisciplinary Treatment Team. The position exercises initiative and independence in setting goals, accomplishing activities, and completing consumer outcomes, while maintaining a professional and responsible program environment under general supervision of the Program Coordinator.

Requirements

High school diploma or GED.
Valid New York State driver’s license.
Six months of experience working with community human service providers and with the developmentally disabled population.
Basic cooking skills.
A combination of training and experience other than that specified may be accepted if judged adequate for the job by the President and CEO.

Bilingual skills; preferred but not required.

Responsibilities

Counsel consumers and provide supervision, training, and assistance with activities of daily living and independent living skills.
Physically assist and coach consumers with toileting and other daily personal hygiene needs.
Maintain up-to-date program notes, outcome notes, and billing for each assigned consumer.
Transport consumers to and from the program daily and, as needed, to activities, including volunteer activities.
Maintain the agency vehicle for safety and submit gas receipts to the Coordinator.
Assist consumers in and out of the van during activities.
Meet fire safety needs, including participating in quarterly fire drills or drills as needed.
Ensure consumer safety at all times.
Keep the facility clean and notify maintenance staff and/or the supervisor of issues.
Attend mandatory training and meetings as scheduled.
Keep all necessary first aid and CPR certifications up to date.
Perform other duties as assigned by the supervisor.
