About The Position

Tyler's Place AZ Inc. is a family-owned agency based out of Chandler, AZ, licensed throughout the entire state of Arizona. As a rapidly growing agency, we are currently seeking providers to work with individuals with developmental disabilities in the home and the community. As a Direct Care Worker, you will be aiding and supporting all age groups by helping to meet their individual goals. We work in a fun and easy-going environment while maintaining a strong work ethic. We are currently looking to add to this dynamic team.

Requirements

MUST PASS AZ DPS Level 1 Fingerprint Clearance application & Criminal History check- State of Arizona requirement
Valid Arizona Driver’s License with clean driving history- State of Arizona Requirement (Willing to transport members)
18+ years of age

Responsibilities

Respite (RSP): Supervise and care for Tyler’s Place AZ members' physical, mental, and/or emotional wellbeing in the members' home or in their community while relieving a family member or person caring for that member.
Attendant Care (ATC): Assist Tyler’s Place AZ members in creating/maintaining a safe, clean living environment, which may or may not include personal cleanliness. Provider will also assist members to engage in community outings and/or activities and to help develop and maintain a daily schedule and/or daily plan of care.
Habilitation (HAH): Assist Tyler’s Place AZ members in a variety of interventions based on the members' specific ISP goals to help them become more independent by improving their daily life and to be a valued member of his/her community. Interventions may or may not include Habilitation therapies, behavior interventions, and/or sensory motor implantation.
