About The Position

We are looking for a Direct Care Worker to provide one-on-one care to patients in their homes, following an individual care plan. The role involves providing personalized, attentive care and building genuine relationships with patients. Candidates should be detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed. Prior Home Health or Hospice experience is a plus. The employer is willing to train candidates.

Requirements

High school diploma or GED.
One year of prior professional experience; the employer is willing to train.
Driver’s license required.
Strong verbal and written communication skills.
Detail-oriented and highly organized.

Responsibilities

Assist patients with daily activities, including moving in and out of beds, baths, wheelchairs, or automobiles.
Change bed linens, do laundry, clean the home, and assist with personal care.
Maintain records of patient care, condition, progress, or problems, and report and discuss observations with the supervisor.
Build relationships with patients through conversation or reading aloud to support their mental health and alertness.
Give medications and immunizations.
Engage patients in exercises or other activities.
Assist consumers with self-administration of medications when they are competent to direct their care.
Assist with housekeeping.
Assist with grooming, dressing, eating, meal preparation, oral hygiene, denture care, toileting, and toilet hygiene.
Provide IADL assistance and emergency first aid.
Provide or arrange social interaction and provide transportation.
