About The Position

The coverage area is Monterey and Santa Cruz. As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.

Requirements

  • Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE)
  • Current licensure in applicable state is required unless employed in the state of AZ, PA or Guam. License must be maintained as current and in good standing.
  • Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
  • Basic computer skills, including Microsoft Office (Word, Excel, Outlook); proficiency in all USRC clinical applications required within 90 days of hire.
  • Meet any practice requirement(s) for the applicable state.

Nice To Haves

  • LCSW preferred
  • Previous experience in providing social services to dialysis patients preferred.

Responsibilities

  • Conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations.
  • Identifying and counseling psychosocial issues and providing patient and family education.
  • Coordinating communities of support for patients and their families, identifying social agencies and other resources (e.g., financial/funding), providing information and referrals, coordinating transient arrangements, and representing your patient as needed with appropriate local, state, and federal agencies.
  • Promoting teamwork, educating staff, and providing training around patient psychosocial care.
  • Participating in all required continuing education and staff meetings.
  • Collaborating with the Medical Director and physicians and maintaining positive relationships with area hospitals, agencies, vendors, and the community.
  • Helping with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes.
  • Participating in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensuring compliance with federal, state, and local laws and regulations.
