About The Position

The Crisis Case Manager is an integral member of the crisis services team, supporting individuals experiencing behavioral health emergencies. This position works across the Crisis Stabilization Unit (CSU), Mobile Crisis, and outpatient-based services, providing treatment planning, discharge coordination, and transitional support. The role includes responsibilities as Shift Charge and coverage for the Crisis Line, ensuring timely triage, coordination, and oversight of urgent needs during assigned shifts.

Requirements

  • Bachelor’s degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education.
  • One year of experience working in mental health/crisis services.
  • Strong oral and written communication skills using the English language.
  • Basic personal computer skills which enable the entry, retrieval, and use of electronic data.
  • A valid driver’s license and automobile liability insurance maintained in accordance with Pawnee policy and procedure.
  • Results of criminal and all other applicable background checks must meet the approval of Pawnee Mental Health Services.

  • General knowledge of mental health, substance use and housing systems of care
  • Experience with Microsoft Office software.
  • Certified in Basic CPR, First Aid, and/or MANDT – if not, will be required to certify at the earliest opportunity.

Responsibilities

  • Collaborate with the interdisciplinary team to develop and implement person-centered treatment and crisis stabilization plans.
  • Work with the Crisis Therapist to support discharge planning, ensuring safe and appropriate community reintegration and follow-up care.
  • Communicate with external providers when appropriate, to coordinate discharge and aftercare supports.
  • Cleaning of the facility, laundry services, meal services, etc.
  • Rounds (15-minute checks), contraband checks, documentation, etc.
  • Provide transportation for clients currently admitted to the CSU or utilizing Crisis Services.
  • Assist in gathering appropriate documentation for the electronic health record, documentation, etc.
  • Link clients to ongoing behavioral health services, substance use treatment, housing, and social supports.
  • Coordinate referrals to transitional housing programs and assist clients with navigating housing applications and requirements.
  • Monitor post-discharge service engagement and facilitate warm handoffs to community providers.
  • Assist with the Crisis intake process, gathering information, supporting documentation, and verifying eligibility criteria.
  • Participate in initial stabilization efforts and coordinate logistics for admissions and transfers.
  • Respond to community-based crisis calls as needed in collaboration with licensed clinicians.
  • Provide immediate case management, safety planning, resource navigation, and family support during field interventions.
  • Serve as point-of-contact and Charge during assigned shifts, ensuring smooth workflow and communication between CSU, mobile, and outpatient crisis staff.
  • Triage needs across the department, prioritize client care tasks, and provide coverage support.
  • Answer incoming crisis calls and conduct phone triage to assess risk, safety, and level of care needs.
  • Provide de-escalation, support, and resource linkage over the phone.
  • Coordinate dispatch of mobile crisis teams or referrals to higher levels of care, including hospitalization if needed.
  • Accurately document all call information in the electronic health record in a timely manner.
  • Complete and stay current on required training for the position.
  • Adhere to the standards, policies and instructions established by PMHS Policy and Procedure and any applicable ethical codes within field of employment.
  • Review and comply with the Pawnee Code of Conduct outlined in the Employee Handbook.
  • Perform all other reasonably related duties as assigned by immediate supervisor or other management personnel.
