Housing and Crisis Case Manager
About The Position
The role is best suited for someone who enjoys building supportive relationships with survivors while helping them achieve long-term housing stability and increased safety. It combines immediate crisis response with ongoing case management, making it both advocacy-focused and service-coordination focused. Unity House is a Rensselaer County-based human service agency that provides a wide range of services to meet the otherwise unmet needs of people in our community who are hurting and struggling. We assist those who are living in poverty, adults living with mental illness or HIV/AIDS, victims of domestic violence, and children with developmental delays. We work to achieve social justice in our community and to create a better understanding of those we serve.
Requirements
- Bachelor’s Degree preferred
- 1 or more years of related experience preferred
- A valid NYS driver’s license and acceptable driving record
- Reliable transportation
Responsibilities
- Provide case management services to domestic and sexual violence victims who reside in our permanent housing program
- Work closely with victims and survivors at varying levels in their trauma
- Provide emotional support, crisis intervention and safety planning
- Provide referrals and linkages to supportive services
- Assist with navigating various systems based on individual needs
- Assist with victim and survivor crisis intervention for “walk ins” in conjunction with appropriate staff.
- Complete contract and/or regulatory compliance activities
- Participate in rotating on-call system that requires some nights and weekends
