About The Position
Rock Valley Community Programs provides transitional and supportive housing, addiction and mental health treatment services to adults in the Rock County area. We provide these services to homeless male veterans; clients connected to the criminal justice system and outpatient care to anyone needing assistance with addiction and/or mental health treatment. Our Crisis Case Manager plays a hands-on role in providing quality care and support to individuals in short-term residential mental health crisis stabilization.
Requirements
- Bachelor’s degree in mental health, human services, or related field preferred
- CNA certification may be considered with relevant experience
- Experience working with adults in crisis or with co-occurring disorders
- Familiarity with trauma-informed care and recovery-based practices
- Strong judgment, problem-solving, and communication skills
- Working knowledge of computers, or willingness to learn
- Valid driver’s license and clean driving record
- Must complete TB test, and drug screening (on-site)
- Completion of CPR, First Aid, CBRF Fire Safety, and Choking training within 90 days
- Employment is contingent upon passing a state and federal background check
Responsibilities
- Provide individualized care that protects consumer rights, dignity, and wellness
- Support individualized crisis stabilization response and service planning
- Facilitate daily individual and group sessions (non-therapeutic)
- Document behaviors, interventions, and progress within 24hours
- Coordinate care and communicate updates with internal crisis team
- Participate in staffings and professional development sessions
- Maintain compassionate, professional relationships with clients and team
- Maintain a minimum billing rate of 66% through timely and accurate documentation
