About The Position

Clearbrook’s CHOICE Libertyville Day Program provides participants with opportunities to continue learning based on their interests and abilities, emphasizing socialization, community participation, and vocational skills for a working environment. The Community Support Coach supports clients’ safety, wellbeing, growth, and development while providing transportation and supervision.

Requirements

• High school diploma or equivalent.
• Valid driver’s license.
• Must be at least 21 years of age.

Responsibilities

• Provide transportation for clients living in Clearbrook’s Community Integrated Living Arrangement (CILA) locations.
• Attend appointments with clients when needed.
• Ensure the safety and wellbeing of clients being transported.
• Supervise clients in general socialization and hygiene skills in accordance with each client’s individualized program plan.
• Take initiative and remain flexible when faced with unexpected situations, problems, tasks, or emergencies.
• Maintain a respectful, friendly, and supportive atmosphere conducive to clients’ growth and development.
