Community Partnerships Coordinator (Mental Health)

The Lorenz Clinic•Minneapolis, MN
•Onsite

About The Position

Lorenz Clinic of Family Psychology is hiring a full-time, salaried Community Partnerships Coordinator to maintain referring relationships across the southwest metro. The clinician-owned mental health clinic serves children, adolescents, adults, couples, and families across Carver, Scott, Dakota, and western Hennepin counties. This is a relationship-stewardship role, not a sales or clinical position. The coordinator maintains a portfolio of referring clinics, practices, schools, and programs; stays in regular contact; gathers feedback; and follows through on commitments. The work is primarily in person and involves calls most mornings and visits to three or four offices most afternoons, with about 25 contacts per week and roughly half conducted in person. The coordinator keeps relationship records current in the clinic’s CRM, without entering client information; provides useful updates and resources to referring offices; and sends a weekly availability bulletin. The role also helps ensure that referred families are scheduled on their first call and, with family permission, that clinician-written clinical summaries are sent at intake, changes in level of care, and discharge. The coordinator arranges conversations between clinic clinicians and referring professionals, records feedback, participates in weekly reviews, and helps prepare monthly reports connecting new clients to referral relationships. Performance is reviewed based on weekly relationship-management work, not referral quotas or commissions. The position is based primarily at a Southwest metro location and is not remote or hybrid. The employee will be closely supervised initially, with less supervision as they take on the work. The stated start date is early November; no year is specified.

  • Provide useful information between requests, such as current openings, answers to prior questions, or resources for families.
  • Send a Monday availability bulletin to referring offices and care navigators.
  • Confirm each morning that families referred the previous day were scheduled on the first call, and raise any exception the same day.
