About The Position

Telecare’s Oregon ACT #204 program is seeking a Nurse Supervisor to provide nursing leadership within a community-based Assertive Community Treatment (ACT) program. The role combines clinical nursing, behavioral health, team leadership, and community-based care. The supervisor leads a small team of three Registered Nurses and works alongside one of two multidisciplinary ACT treatment teams, coordinating with clinicians, case managers, prescribers, and other behavioral health professionals. The four-person nursing team supports members in remaining successful in the community. This is a 40-hour salaried position with a typical Monday–Friday schedule of 8:30 AM–5:00 PM. A scheduled leadership on-call rotation is part of the role; rotations are arranged in advance, and the supervisor must remain available to respond to program needs during their assigned rotation. Because ACT services are community based, the role includes providing services and support in a variety of community settings. Assertive Community Treatment provides intensive, coordinated support in the community for individuals with significant behavioral health needs. Nurses work alongside behavioral health professionals to support medication management, physical and behavioral health needs, recovery goals, community stability, and overall wellness. The role offers continuity of care and opportunities to build longer-term relationships with members served. The expected starting salary is $118,995.46–$134,000.00 annually. Telecare applies geographic differentials, and the assigned pay range depends on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
