Community Partnerships Coordinator (Mental Health)

The Lorenz Clinic, LLC•Minneapolis, MN
•$55,000 - $62,000•Onsite

About The Position

The Community Partnerships Coordinator manages and maintains a portfolio of referring offices—including clinics, practices, schools, and programs—in the southwest metro. The role is centered on consistent relationship stewardship, clear communication, and reliable follow-through rather than sales. The coordinator contacts partners on a schedule, typically making calls in the mornings and visiting three or four offices most afternoons, and sends a weekly availability bulletin on Mondays. They gather partner feedback at every contact, record it, and bring themes to a weekly review. They also keep accurate relationship records in the CRM, without entering client information, and provide useful updates and resources between referral requests. The coordinator helps ensure referred families are scheduled on the first call, follows up promptly when they are not, and sends and records clinician-written clinical summaries with the family's permission at intake, changes in level of care, and discharge. The role also arranges conversations, talks, and workshops between Lorenz Clinic clinicians and referring professionals or their communities. The coordinator contributes weekly contact counts and helps prepare monthly reporting that connects new clients with referring relationships. This is a full-time, salaried, in-person position, housed primarily at a southwest metro clinic location. The start date is in early November.
