About The Position

Reema Health is seeking a bilingual English/Spanish Community Health Navigator to conduct outreach and enroll members in Reema’s care model. The Navigator builds and manages a caseload of 80–120 members, assesses member needs—including social determinants of health—and develops member-centered care plans. The role focuses on building trusting relationships, helping members work toward their goals, addressing barriers to care, coordinating referrals and appointments, and connecting members with community and health resources. This may include transporting or accompanying members as needed. The Navigator manages their workload independently, meets monthly goals and daily metrics, and engages members through phone, text, email, and in-person visits. They document work and keep care plans current in the Reema App, protect confidential information, and consult the multidisciplinary team on complex situations. The role requires adaptability and collaboration in a fast-paced, changing startup environment, as well as occasional required in-person team meetings despite the remote work environment. Responsibilities may change as business needs evolve. This is emotionally demanding, community-based work that involves supporting people through hardship and potential rejection. Candidates must be available Monday through Friday during core business hours, and offers are contingent on passing a pre-employment drug screening and background check.
