About The Position

The Community Health and Outreach Project Coordinator is an integrated role that supports both the hospital’s community health responsibilities and outreach program initiatives. This position plays an important role in the execution of the Community Health Needs Assessment (CHNA) and implementation of the Community Health Improvement Plan (CHIP). Working collaboratively across departments, the specialist partners with clinical teams, Dartmouth Health colleagues, and community-based organizations to help identify and respond to community health needs.

Requirements

• Excellent communication, analytical, interpersonal skills.
• Ability to work collaboratively with diverse teams and stakeholders.
• Strong organizational and project management skills.
• Demonstrated understanding of community-based population health initiatives.
• Bachelor degree in a related field or equivalent experience.
• Experience in a healthcare, social work, population health, community health or a related field required.

Responsibilities

• Serves as the point of contact between Dartmouth Health and New London Hospital to support community health and outreach initiatives and facilitate collaboration.
• Coordinates community health programs and outreach activities by assisting with, scheduling, program logistics and stakeholder engagement.
• Maintains relationships with clinical teams, community partners and supports collaborative efforts that improve community health and wellbeing.
• In partnership with the development team, assists with the administration and distribution of funding to support programs that address social determinants of health and advance community health priorities.
• Supports the tracking and reporting of community benefit activities for mandated reporting purposes.
• In partnership with Dartmouth Health, community-based organizations, and other service area partners supports the implementation and ongoing monitoring of the Community Health Needs Assessment (CHNA) and implementation of the Community Health Improvement Plan (CHIP) on a three-year cycle.
• Coordinates the collection and tracking of program outcomes and success stories and assists in communicating the impact of community health initiatives and outreach activities to internal and external stakeholders.
• Take on additional responsibilities as needed to support the team and organizational goals
