About The Position
Allcare Home Health Agency is seeking an organized, dependable, and experienced Certified Nursing Assistant (CNA) to serve as a Facility Coordinator at an independent living facility in Chapel Hill. The coordinator oversees and organizes assigned home care services for clients living in independent and assisted living facilities and serves as the primary liaison among the agency, facility staff, caregivers, clients, and families. The role supports effective communication, appropriate staffing, quality service delivery, client safety, and client satisfaction. The position includes coordinating caregiver assignments and schedules, monitoring attendance and service delivery, conducting client check-ins, and assisting with personal care tasks. The coordinator communicates concerns and changes in client condition to the agency’s Director of Nursing or other appropriate clinical personnel. Requirements
- Current North Carolina CNA certification.
- At least 5 years of experience working as a CNA.
- Excellent organizational and time-management skills.
- Excellent communication and interpersonal skills.
- Compassionate, dependable, and professional demeanor, with the ability to establish positive working relationships.
- Ability to use computers, email, scheduling systems, and electronic documentation platforms.
- Must successfully complete applicable background checks and agency screening.
- Current TB test.
- BLS certification from the American Heart Association.
- Reliable transportation and a valid driver’s license.
- Current auto insurance.
Nice To Haves
- High school diploma or equivalent preferred.
- Experience working in assisted living or independent living facilities preferred.
- Previous supervisory, scheduling, or coordination experience is a plus.
Responsibilities
- Serve as the primary agency contact at the facility.
- Build and maintain positive working relationships with facility administrators, directors of nursing, and other facility personnel.
- Monitor service delivery and maintain communication with facility partners.
- Address facility concerns and coordinate timely resolutions.
- Promote agency services and maintain professional relationships with facility partners.
- Coordinate caregiver assignments and staffing schedules for clients in assigned facilities.
- Monitor caregiver attendance, punctuality, and shift coverage.
- Assist with caregiver orientation and facility-specific expectations.
- Report staffing concerns and performance issues to management.
- Assist clients with bathing, dressing, grooming, toileting, and other personal care.
- Provide medication reminders, follow individualized care plans and agency policies, and check vital signs.
- Conduct routine client check-ins and service-satisfaction visits.
- Monitor whether services are being provided according to the approved service plan.
- Communicate client concerns, complaints, and changes in condition to the agency’s Director of Nursing or appropriate clinical personnel.
- Promptly refer concerns requiring clinical assessment to the Director of Nursing or designated clinical personnel.
- Maintain regular communication with clients, families, and facility representatives.
- Promote client safety, dignity, and satisfaction.
