In-Home Health - NP or PA (Part Time) - Boulder, CO
About The Position
Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order labs, or alter current treatment plans. The primary workplace is the private residence of health plan members, which can be diverse and may contain household pets, secondhand smoke, or airborne allergens.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Able to cover the following primary locations: Boulder County.
Nice To Haves

Responsibilities
- Provide In-Home Health Evaluations.
- Engage with people in the comfort of their own homes.
- Bridge gaps in care.
- Build trusted relationships to make people healthier.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
- Be flexible to travel locally and within licensed states, as needed.
