About The Position

Concierge Home Care is hiring a Clinical Team Support (Scheduling Coordinator) to support daily home health clinical operations. The role coordinates patient and clinician schedules, facilitates communication between patients and clinicians, and helps ensure patients receive care according to physician orders and care plans. It is a fast-paced, team-oriented, in-office position in Sarasota, Florida, suited to someone with prior home health scheduling experience.

The schedule is Monday through Friday, 8:30 AM to 5:00 PM, for 40 hours per week. Compensation is negotiable based on experience. The role may require successful completion of an AHCA Level 2 background screening.

Requirements

• High school diploma.
• Prior home health scheduling experience.
• Strong organizational and multitasking skills.
• Ability to work in a fast-paced, team-driven environment.
• Excellent communication and problem-solving abilities.

Responsibilities

• Schedule patient visits in accordance with physician orders and care plans.
• Coordinate clinician schedules to optimize productivity and coverage.
• Manage daily scheduling changes, call-outs, and urgent visit needs.
• Answer and triage incoming calls to support clinical operations.
• Track medical supplies and support RAP/EOE processes.
• Maintain accurate scheduling and documentation within the EMR.
• Support the clinical team with administrative tasks as needed.
