We are seeking an experienced, energetic, and compassionate Registered Nurse (RN) to join our leadership team as a Clinical Services Manager, RN – Home Health. This is an outstanding opportunity for a home health leader who enjoys mentoring clinicians, overseeing patient care operations, and making a meaningful impact on both patients and staff. The Clinical Services Manager is responsible for the day-to-day management and supervision of clinical operations and multidisciplinary home health teams. This position ensures timely, high-quality patient care, supports clinician development, promotes regulatory compliance, and assists in driving excellent patient outcomes.

Requirements

Current Illinois Registered Nurse (RN) license in good standing.
Minimum of two years of supervisory or management experience in a Medicare-certified Home Health Agency.
Strong knowledge of Medicare Conditions of Participation and home health regulations.
OASIS knowledge and documentation experience.
Excellent clinical assessment and critical-thinking skills.
Strong leadership, communication, and organizational abilities.
Excellent computer and electronic medical record proficiency.
Ability to manage multiple priorities in a fast-paced environment.
Recent Medicare-certified Home Health leadership experience.

Responsibilities

Provide daily supervision and support to Registered Nurses, Licensed Practical Nurses, Home Health Aides, Physical, Occupational and Speech Therapists, and Medical Social Workers.
Coordinate and monitor clinical operations and patient care delivery.
Conduct case conferences and assist with care planning and coordination.
Review clinical documentation, OASIS assessments, and physician orders for accuracy and compliance.
Monitor patient outcomes, clinician productivity, and quality indicators.
Provide education, coaching, and mentoring to clinical staff.
Assist with quality assurance activities and survey readiness initiatives.
Serve as a clinical resource to staff, patients, physicians, and referral sources.
Promote teamwork, accountability, and exceptional customer service.
Assist in developing and implementing clinical policies and procedures.
