Clinical Nurse Liaison (MGH Campus) Help More Patients Access the Care They Need Care Dimensions is seeking a relationship-driven Clinical Nurse Liaison to serve as the primary ambassador for Care Dimensions on the Massachusetts General Hospital campus, including MGH, the MGH Cancer Center, and the new cancer building. This role is dedicated to building strong partnerships with case managers, clinicians, and healthcare teams to increase awareness of Care Dimensions services and facilitate referrals for patients who may benefit from hospice care at home or in one of our Hospice Houses. This position focuses on relationship development, education, and referral growth by helping healthcare partners understand when and how to refer patients to hospice. This is a full-time, 40-hour position with flexibility in scheduling, including potential weekend coverage based on candidate availability and business needs.

Requirements

RN or LPN licensure in Massachusetts preferred.
Experience in healthcare relationship management, clinical liaison work, care coordination, business development, or community outreach preferred.
Exceptional communication, presentation, and relationship-building skills.
Ability to work independently while collaborating effectively across teams.
Passion for improving access to compassionate end-of-life care.

Responsibilities

Develop and maintain strong relationships with case managers, physicians, nurses, and healthcare leaders across the MGH campus.
Serve as a trusted resource on hospice services, eligibility, care options, and Care Dimensions programs.
Increase awareness of home hospice and Hospice House services through education and consultation.
Create and execute a territory engagement strategy to strengthen partnerships and support organizational growth.
Collaborate with internal teams to ensure a seamless referral experience for patients and families.
Monitor referral trends and identify opportunities to improve access to hospice care.
