About The Position
Care Dimensions is seeking a Per Diem Clinical Nurse Liaison to work at a large academic medical center in Boston. The role helps patients access hospice care at the right time by providing hospice education, evaluating patients for hospice eligibility, and connecting patients and families with care and support. The liaison works closely with case managers, physicians, nurses, and other healthcare professionals. The position offers flexible workdays and hours, job-share opportunities, and the potential to grow into a full-time position.
Requirements
- Experienced nurse with clinical expertise.
- Communication skills and the ability to educate others.
- Relationship-building experience.
Responsibilities
- Provide hospice education to healthcare professionals, patients, and families.
- Evaluate patients for hospice eligibility.
- Help connect patients and families with the care and support they need.
- Work closely with case managers, physicians, nurses, and other healthcare professionals.
- Build relationships with healthcare professionals and support appropriate, timely access to hospice care.
