About The Position

Care Dimensions is hiring a Per Diem Clinical Nurse Liaison to work at a large academic medical center in Boston. The role helps patients access hospice care at the appropriate time and involves collaborating with case managers, physicians, nurses, and other healthcare professionals. The liaison provides hospice education, evaluates patients for hospice eligibility, and helps connect patients and families with care and support. The position offers flexible workdays and hours, job-share opportunities, and potential to grow into a full-time position.

Responsibilities

• Provide hospice education to healthcare professionals, patients, and families.
• Evaluate patients for hospice eligibility.
• Help connect patients and families with appropriate care and support.
• Work closely with case managers, physicians, nurses, and other healthcare professionals.
