About The Position

Signet Health is seeking a Clinical Liaison for the 10-bed inpatient rehabilitation unit at CommonSpirit Holy Cross Hospital in West Jordan, Utah. The Clinical Liaison conducts pre-admission assessments in the hospital and community-based facilities and provides education to referral entities and community education groups.

Requirements

Current Utah state licensure in nursing (RN) or an allied health profession (PT, OT, or SLP).

Nice To Haves

Experience as a clinical liaison in acute rehabilitation or a closely related field.

Responsibilities

Respond to requests for assessments and prioritize internal inquiries to expedite access to the unit.
Proactively seek potential referrals from charge nurses, medical directors, and case managers.
Communicate daily with the Medical Director and Director of Rehabilitation about referrals, admissions, and discharges, and enter required information in SigNetwork.
Develop and maintain positive relationships with host hospital employees and Signet team members.
Identify external referral sources, schedule on-site meetings, and educate referral sources on unit admission criteria and intake.
Provide ongoing and follow-up communication to referral sources, patients, families, nursing staff, the Medical Director, and therapists.
Adhere to hospital and Signet Health policies and procedures, ethical standards, corporate compliance standards, and the professional code of conduct.
