About The Position

Signet Health is seeking a Clinical Liaison for the acute rehabilitation unit at Community Regional Medical Center in Fresno, California. The Clinical Liaison conducts pre-admission assessments at area hospitals and other local health care facilities. Working primarily outside the hospital, the liaison identifies patients who may need acute rehabilitation, performs on-site clinical assessments, and builds trusted relationships with referring physicians, case managers, and hospital teams.

Requirements

Current California licensure in nursing (RN) or an allied health profession (PT, OT, or SLP).
Three years of experience as a clinical liaison in rehabilitation or a closely related field.

Nice To Haves

RN licensure is preferred.

Responsibilities

Build trust with patients, families, physicians, and all referral sources.
Proactively seek referrals from hospitals, assisted living centers, home health providers, physicians, and case managers.
Communicate daily with the ARU Medical Director and Director of Rehabilitation regarding referrals, admissions, and discharges, and enter required information in UDS.
Develop and maintain positive relationships with hospital employees and Signet team members.
Present rehabilitation education and unit criteria to case managers, physicians, and other referring entities through proper channels.
Provide ongoing, follow-up, and discharge communication to referral sources, patients, families, nursing staff, the medical director, and therapists.
Follow hospital and Signet Health policies and procedures, ethical and corporate compliance standards, and the professional code of conduct.
