About The Position

Select Specialty Hospital Critical Illness Recovery Hospital (LTACH) is seeking a Per-Diem Clinical Liaison (RN/RRT) to advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. This role serves as a resource for healthcare professionals by fostering relationships within medical facilities. With clinical expertise and problem-solving skills, the liaison coordinates care plans with physicians and families, facilitates patient transfers, and addresses barriers to accessing specialized post-ICU care. The rate for this position is $42/hr, and the work schedule is on an as-needed basis for weekend coverage, PTO, and medical leave coverage.

Requirements

  • Current and valid state licensure or eligibility for state licensure as a Registered Nurse or Respiratory Therapist.
  • Demonstrated computer proficiency.
  • Must exercise sound judgment in handling the professional/confidential nature of healthcare sales.

Nice To Haves

  • Two years of related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role.
  • Demonstrated competency with the preadmission process, including assessments and relationship building in a rehabilitation setting or in another post-acute care setting.
  • Proven track record in sales and marketing with at least one year of experience in a related position.

Responsibilities

  • Advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs.
  • Serve as a resource for healthcare professionals by fostering relationships within medical facilities.
  • Coordinate care plans with physicians and families.
  • Facilitate patient transfers and address barriers to accessing specialized post-ICU care.
  • Work with physicians, discharge planners, and patients' families to plan the best continuum of care after an ICU or post-surgical stay by utilizing strong clinical background, payer knowledge, and problem-solving skills.
  • Perform clinical assessments of referred patients, then counsel with patients' families to assist them in making the best care decisions for their loved one.
  • Partner with hospital case managers and physicians to create a smooth pre-admission and transfer process by removing payer-related and operational barriers for patients getting into our highly effective level of care.
  • Cover some weekends, PTO, and medical leave for full-time employees.
