About The Position

The CL (Clinical Liaison) is responsible for assessing potential residents/guests in the hospital to establish eligibility for care and screen for appropriateness of placement. This position is also responsible for identifying, building, and maintaining key relationships with physicians, discharge planners, senior living providers, home health agencies, and other healthcare providers. The role involves understanding business problems and opportunities, evaluating potential patients, assessing and communicating patient needs, and making observations about market changes to develop action plans. The Clinical Liaison participates in and contributes to meeting occupancy levels according to the marketing plan, develops positive relationships with various healthcare professionals and families, and actively communicates with prospective residents/guests to generate activity through frequent personal contact. They educate stakeholders on program services and resources, coordinate with staff for smooth discharge, communicate admission criteria and payer information, obtain necessary medical information, and ensure accurate clinical admission records are obtained. Information about the prospective resident/guest is transmitted to the Admissions Coordinator. Additional duties may be assigned.

Requirements

  • Associates degree required.
  • Knowledge of the information and techniques needed to assess and treat human injuries and diseases, including symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
  • Effective verbal and written communication skills.
  • Strong presentation skills with ability to adjust accordingly for a variety of audiences.
  • Interpersonal skills including the ability to form constructive and effective relationships with clients, families, and customers.
  • Excellent customer service skills.
  • Knowledge of regulatory standards, compliance requirements, and hospitals policies and procedures.
  • Demonstrate ability to manage territory and accounts.
  • Patient care experience.
  • Clean Driving Record.
  • Valid driver’s license.

Nice To Haves

  • Previous discharge planning experience preferred.
  • Sales/Marketing experience preferred.
  • RN, LPN, RRT or paramedic preferred.

Responsibilities

  • Assessing potential residents/guests in the hospital to establish eligibility for care and screen for appropriateness of placement.
  • Identifying, building, and maintaining key relationships with physicians, discharge planners, senior living providers, home health agencies, and other healthcare providers.
  • Understanding business problems and opportunities and recommending solutions.
  • Evaluating potential patients.
  • Assessing and communicating patient needs with families, referring CM’s, and referring physicians.
  • Making observations about changes within the market and assisting in making recommendations and creating action plans.
  • Participating in, developing, and meeting the occupancy levels in accordance with the marketing plan.
  • Developing positive relationships with discharge planners, physicians, families, senior living providers, home health agencies, and other healthcare providers, marketing company programs and services.
  • Actively communicating with prospective residents/guests, family members, and key influencers to generate activity.
  • Assessing potential residents/guests, reviewing their medical records, and determining whether they should be admitted for care in collaboration with physicians, nurses, therapists, social workers, case managers, and other interdisciplinary team members.
  • Educating potential residents/guests, family members, referral sources, and external payers regarding the effective and efficient utilization of program services and available resources.
  • Coordinating with other staff members to ensure a smooth discharge process.
  • Communicating company admission criteria and obtaining payer type information such as Medicare, Managed Care, and Private Pay.
  • Obtaining current medication and treatment information and the potential resident’s/guest’s health care needs.
  • Ensuring that clinical admission records are obtained accurately from other healthcare facilities.
  • Transmitting information relating to the prospective resident/guest to the Admissions Coordinator.
