We are looking to grow our team with a Full-time RN or LPN Clinical Liaison that shares our passion for creating the best experience for our Health Care Center residents. The Clinical Liaison serves as a key representative of the skilled nursing facility, responsible for developing and maintaining strong relationships with hospitals, physicians, case managers, and community partners to support admissions growth. This role facilitates the clinical review of potential referrals to ensure appropriate placement and acts as a backup to the Director of Admissions, ensuring continuity of the admissions process.

Requirements

  • Unrestricted Virginia Licensed Nurse (LPN or RN) preferred OR Bachelor’s degree in healthcare, marketing, or related field (with relevant experience)
  • 2–3 years of experience in skilled nursing, hospital discharge planning, case management, or healthcare marketing.
  • Strong clinical assessment skills (if licensed)
  • Excellent communication and relationship-building skills
  • Ability to multitask and manage a fast-paced environment
  • Strong organizational and problem-solving abilities
  • Knowledge of EMR systems and Microsoft Office

Nice To Haves

  • Prior admissions or liaison experience preferred.
  • Knowledge of Medicare, Medicaid, managed care, and PDPM preferred.

Responsibilities

  • Develop and maintain relationships with hospital discharge planners, case managers, social workers, and physicians.
  • Conduct routine hospital visits to generate referrals and promote facility services.
  • Serve as the primary point of contact for assigned referral sources.
  • Represent the facility at community events and networking opportunities.
  • Participate in on-call rotation for admissions as needed.
  • Support facility census goals and strategic initiatives.
  • Maintain a professional image representing the organization.
