About The Position

WellPower is hiring a Clinical Case Manager to provide comprehensive psychological and behavioral case management services for people living in the organization’s Permanent Supportive Housing (PSH) units. The role serves primarily people receiving high-intensity treatment team (HITT) services, with occasional assignments at the community treatment team (CTT) level. Case managers are the main point of contact for people on their caseloads, which average 15 people and may change with agency needs. The role is primarily community based, with the case manager assigned to and based out of a PSH building. Support may also be provided as people transition into or out of housing. The case manager may conduct eligible intakes in various settings. The position assesses and follows up on people’s ability to meet basic needs and move toward recovery, and is responsible for clinical documentation of each interaction. Case managers help people access services and community resources, including by transporting them in the case manager’s personal vehicle. WellPower reimburses the additional cost of business-use auto insurance and mileage. The position carries a grant to help support caseload members’ basic needs. The position is available at Clinical Case Manager I (bachelor’s degree) and Clinical Case Manager II (master’s degree) levels. WellPower states that salary decisions are based on relevant factors such as experience, education, and certification or licensure. Full-time, benefits-eligible employees qualify for the benefits plan. The posting also describes benefits and employee programs including paid holidays, medical, dental and vision insurance, a retirement match, gainsharing, free licensure supervision for specified clinicians, employee resource groups, and sports leagues.
