About The Position

Roze Room is hiring a full-time, field-based Clinical Care Coordinator (LVN) serving the Culver City and Greater Los Angeles areas. The coordinator acts as a liaison among patients, families, referral sources, and the hospice and palliative care team. The role provides education, conducts nursing assessments, coordinates admissions, and helps ensure patients receive timely, compassionate care.

Requirements

• Current California LVN license.
• At least 1 year of nursing experience.
• Strong communication skills.
• Reliable transportation and a valid driver’s license.

Nice To Haves

• Hospice experience is highly preferred.
• Bilingual Spanish is preferred.

Responsibilities

• Conduct nursing assessments and admissions.
• Review consents and other documents and assist with election of the hospice benefit.
• Educate patients and families about hospice and palliative care.
• Coordinate care transitions from hospitals and facilities.
• Collaborate with interdisciplinary teams.
• Complete documentation using EMR systems.
