About The Position

The Bilingual LVN Care Coordinator is an essential member of Titanium Healthcare’s Enhanced Care Management (ECM) team. ECM is a person-centered program funded through California Advancing and Innovating Medi-Cal (CalAIM), designed to coordinate medically appropriate, cost-effective care and improve members’ social and physical determinants of health. Potential members must be enrolled in Inland Empire Health Plan (IEHP). The team conducts outreach and engagement, enrolls members in the program, and provides in-person care management and coordination for people who frequently use health services and individuals experiencing homelessness. Members remain enrolled until they no longer need or want services. This is a hybrid, field-based position. Approximately 80% of the work is in-person service to members in the community, and approximately 20% is remote administrative, documentation, and other job-related work. The role requires regular member engagement and travel within the assigned service area. The employee has control over their schedule when meeting members and is eligible for mileage reimbursement for business-related vehicle use. Standard business hours are Monday through Friday, 8:30 a.m. to 5:00 p.m.
