About The Position

The LVN Care Coordinator is an essential member of Titanium Healthcare’s Enhanced Care Management (ECM) team. ECM is a person-centered program funded through California Advancing and Innovating Medi-Cal (CalAIM) that coordinates care to improve members’ health and life trajectory. The program serves eligible members enrolled in Inland Empire Health Plan (IEHP), including frequent users of health services and people experiencing homelessness. The team provides in-person care management and coordination, connects members with managed care providers and community resources, and supports improvements in social and physical determinants of health. Members remain in the program until they no longer need or want its services. This is a hybrid, field-based position. Approximately 80% of the time is spent providing in-person services to members in the community, with approximately 20% spent on remote administrative, documentation, and other job-related duties. The role requires regular in-person member engagement and travel within the assigned service area. The employee controls their schedule when meeting members and is eligible for mileage reimbursement for business-related vehicle use. Standard business hours are Monday through Friday, 8:30 a.m. to 5:00 p.m.
