About The Position

The LVN Care Coordinator is an essential member of Titanium Healthcare’s Enhanced Care Management (ECM) team. The California Advancing and Innovating Medi-Cal (CalAIM)-funded ECM program uses a person-centered approach to coordinate medically appropriate, cost-effective care and improve members’ social and physical determinants of health. Potential members must be enrolled in Inland Empire Health Plan (IEHP). Titanium conducts outreach and engagement, enrolls members, and coordinates care for people who frequently use health services and individuals experiencing homelessness. Services include in-person care management and coordination with managed care providers and community organizations. Members remain enrolled until they no longer need or want services. This hybrid, field-based role involves spending approximately 80% of time providing in-person services in the community and approximately 20% on remote administrative, documentation, and other job-related duties. The coordinator travels regularly within an assigned service area and has control over their schedule when meeting members. Mileage reimbursement is available for business-related vehicle use. Standard business hours are Monday through Friday, 8:30 a.m. to 5:00 p.m.
