Bilingual LVN Care Coordinator

Titanium Healthcare•Indio, CA
•$24 - $33•Hybrid

About The Position

The LVN Care Coordinator is an essential member of Titanium Healthcare’s Enhanced Care Management (ECM) team. ECM is funded through California Advancing and Innovating Medi-Cal (CalAIM) and uses a person-centered approach to coordinate care and improve members’ health and life trajectories. Potential ECM members must be enrolled in Inland Empire Health Plan (IEHP). Titanium conducts outreach and engagement, enrolls members, and provides services intended to improve social and physical determinants of health. Members remain in the program until they no longer need or want services. ECM care teams provide in-person care management and coordination, particularly for frequent users of health services and people experiencing homelessness. This is a hybrid, field-based position. Approximately 80% of work is spent providing in-person services in the community, and approximately 20% is spent on remote administrative, documentation, and other job-related duties. The role requires regular member engagement and travel within the assigned service area. The employee controls their schedule when meeting members and may receive mileage reimbursement for business-related vehicle use. Standard business hours are Monday through Friday, 8:30 a.m. to 5:00 p.m. The position coordinates access to healthcare and community resources, supports members’ health behaviors, and works with an interdisciplinary ECM team to provide care management. The LVN Care Coordinator helps ensure smooth transitions after emergency room visits and hospitalizations, completes follow-ups and assessments, initiates care plans, and provides health education within the scope of practice. The role also supports other ECM team members and follows company and departmental policies, consulting the ECM Operations Leader and assigned POD RN before taking clinical action.
