Bilingual LVN Care Coordinator

Titanium Healthcare•Victorville, CA
•$24 - $33•Hybrid

About The Position

The LVN Care Coordinator is an essential member of Titanium Healthcare’s Enhanced Care Management (ECM) team. ECM is a person-centered program funded through California Advancing and Innovating Medi-Cal (CalAIM) that coordinates medically appropriate, cost-effective care for eligible Inland Empire Health Plan (IEHP) members, including frequent users of health services and people experiencing homelessness. Titanium conducts outreach and enrollment and provides services intended to improve members’ social and physical determinants of health. Care teams provide in-person care management and coordination, connecting members with services from managed care providers and community organizations. Members remain in the program until they no longer need or want its services. This is a hybrid, field-based position. Approximately 80% of the work is in person with members in the community, and approximately 20% is remote administrative, documentation, and other job-related work. The role requires regular in-person member engagement and travel within the assigned service area. The coordinator has control over their schedule for member visits and is eligible for mileage reimbursement for business-related use of their vehicle. Standard business hours are Monday through Friday, 8:30 a.m. to 5:00 p.m.
