Titanium Healthcare•San Bernardino, CA
•Hybrid

About The Position

The LVN Care Coordinator is an essential member of Titanium Healthcare’s Enhanced Care Management (ECM) team. ECM is funded through California Advancing and Innovating Medi-Cal (CalAIM) and uses a person-centered approach to coordinate care and improve members’ health and life trajectory. The program serves enrolled Inland Empire Health Plan (IEHP) members, including frequent users of health services and people experiencing homelessness. The team provides in-person care management and coordination, supports access to managed care providers, and works to improve social and physical determinants of health. Members remain enrolled until they no longer need or want the services. This is a hybrid, field-based position. Approximately 80% of the work is providing in-person services to members in the community, and approximately 20% is remote administrative, documentation, and other job-related work. The role requires regular in-person member engagement and travel within the assigned service area. The employee controls their schedule when meeting members and is eligible for mileage reimbursement for business-related vehicle use. Standard business hours are Monday through Friday, 8:30 a.m. to 5:00 p.m.
