About The Position
The Clinical Care Coordinator communicates effectively with patients, nurses, and providers and triages physicians and patients using clinical understanding. The role works with outside providers, locations, and agencies to ensure coordination of care.
Requirements
- High school diploma or equivalent required.
- Ability to communicate effectively with patients and physicians.
