We are seeking a Clinical Assessor Delegate to deliver end-to-end aged care assessments within the My Aged Care framework, combining clinical expertise with autonomous decision-making and strong operational performance. Reporting to the Clinical Assessor Team Lead, the Clinical Assessor Delegate is responsible for delivering aged care assessment, triage and delegated decision-making functions within the My Aged Care framework. The role undertakes Comprehensive Assessments, Home Support Assessments and Support Plan Reviews (SPRs) using the Integrated Assessment Tool (IAT), determines eligibility and support needs, and exercises approved Assessor Delegate authority to finalise service approvals and funding recommendations in line with legislative and contractual requirements. This is a dynamic, community-based role where you will manage your own caseload, perform triage functions, and work both independently and collaboratively to meet key performance targets.

Requirements

  • Tertiary qualification in Nursing, Occupational Therapy, Physiotherapy or Social Work
  • Current AHPRA registration
  • Minimum 1 year experience in aged care assessments or geriatric services
  • Completion (or willingness to complete) My Aged Care training and delegate requirements
  • Proven ability to work in KPI-driven environments
  • Strong communication skills with a client-centred approach
  • Solid IT skills across multiple systems and Microsoft Office
  • Current driver’s licence and ability to meet compliance checks (NDIS, Police Check, Blue Card, First Aid/CPR)

Responsibilities

  • Deliver high-quality, person-centred aged care assessments across community and remote settings
  • Review and triage referrals, ensuring timely progression within KPI timeframes
  • Complete accurate documentation and support plans within 24–48 hours
  • Exercise delegation authority to finalise service approvals and funding decisions
  • Manage an active caseload while maintaining productivity benchmarks
  • Maintain ≥95% documentation accuracy and 100% compliance with program guidelines
  • Identify and escalate risks impacting service delivery
  • Contribute to quality improvement, audit, and governance activities
