About The Position

CLFC Healthcare and Communications is hiring a per diem Certified Nursing Assistant (CNA) for Wayne County's 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The on-site role pays $25.00 to $28.00 per hour, with experience affecting the rate and night and weekend shifts paid at the top of the range. Shifts are selected by the CNA through the BRIDGE App, with no minimum hours; available shifts include eight-hour shifts and, when available, twelve-hour shifts. The facility operates day, evening, and overnight shifts. Overtime over 40 hours and work on all ten County holidays are paid at time and a half. Confirmed shifts canceled by the facility two hours or less before their start are paid for four hours. Pay is weekly, and orientation is paid at the employee's rate. The CNA provides residents with personal care, meal assistance, mobility support, vital-sign monitoring, and observation and reporting of changes in condition. The role includes supporting safe movement with proper body mechanics and lift equipment, documenting vital signs and other measurements, and maintaining a clean, safe environment while following infection-control practices. The facility is described as High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has licensed nurses on every shift. The position includes reimbursement for CPR renewal, support for continuing education and certifications, and up to $1,500 per year in tuition assistance toward an LPN or RN program. A $500 referral bonus is offered for referring a nurse or aide who joins the roster. Fingerprinting, immunizations, screening, and license verification are arranged and paid by CLFC; paid sick leave accrues under New York law, and professional liability insurance covers each shift. The role is W-2 per diem. CLFC describes its program terms as provided in writing before acceptance and offers a Program Manager as a contact for scheduling and pay questions.
