About The Position

Suncrest At Brighton/Suncrest Hospice aims to change expectations for hospice care by providing exceptional care and service to patients. The organization increases staff-to-patient ratios to support more clinical visits and lower clinician caseloads and is a Community Health Accreditation Partner (CHAP)-certified hospice.

The Certified Nursing Assistant/Home Health Aide (CNA/HHA) provides care to assigned residents under the direction of a Case Manager/DON, following resident assessments, care plans, and nursing management direction. The position coordinates with other licensed care staff to ensure appropriate care and adheres to company nursing policies, procedures, and safety guidelines. The schedule is Monday through Friday, 6:00 a.m. to 2:30 p.m.

Requirements

• Commitment to improving quality of life and partnering with community organizations in patient care.
• Ability to work well in a team environment and collaborate with nurses, social workers, chaplains, volunteers, and Aide leadership.
• Ability to work independently on a day-to-day basis.
• Willingness to drive, with reliable transportation, a valid driver’s license, and auto insurance.
• Certified Nursing Assistant (CNA) certification in the state(s) of practice.
• Home Health Aide (HHA) certification in the state(s) of practice.

Responsibilities
